Columbus Park Nursing Center is located in the Austin neighborhood of Chicago on the border of the town of Oak Park. Our home is approved for Medicare, Medicaid, and private insurances. We also accept Public Aid pending referrals with no deposit required. Columbus Park Nursing Center provides skilled and intermediate nursing care to those individuals whose needs range from IV's to respiratory therapy, from wound care to hospice and respite care, from PT, OT, and speech therapy to Dementia care. Our recreation department offers a wide variety of fun, stimulating activities for the mind, body, and soul. Our food service is second to none. About Us
Conveniently located on Chicago's Westside and serving the needs of Chicago and neighboring suburbs is Columbus Park Nursing and Rehabilitation Center. We are a 216 bed skilled nursing facility licensed by the state of Illinois and approved and certified for Medicare, Medicaid, Medicaid pending and many insurance plans.
Our commitment to caring and quality care starts with our employees. We seek out the best nurses, therapists, aides and others to join our caring staff - our family. Our consultant Physicians are on staff at many of the area hospitals. Each member of our teams strives toward ensuring that every resident of our home receives the best care and comfort possible.
Columbus Park Nursing and Rehab Center is comprised of four resident floors that can accommodate almost any level of care. The fourth and fifth floors are designed to meet the needs of residents who need minimal assistance. The second and third floors are designed for individuals requiring skilled nursing care or skilled rehabilitation services. Our certified Medicare units (hospital step-down) are located on the second and third floors. The third floor is designed to meet the needs of and provide care primarily to individuals with Alzheimer's disease and associated illnesses. Our first floor hosts two dining rooms and public areas.
Whether short term rehabilitation or long term care is needed, our staff will design a program to meet each individual's needs. We recognize that reaching and maintaining the highest level of independence and physical fitness provides many benefits toward overall well-being.
Individualized Care is an important aspect of healing and rehabilitation. All of the residents in our home receive a plan of care to meet their own particular needs and goals. Residents and family members are encouraged to participate in the Care Plan process. Columbus Park Nursing and Rehab Center works with many of the area hospitals to provide a continuum of care to our community.
